Bagong Philippine Passport Act, pinapayagan ng kababaihan na muling gamitin ang maiden name sa kanilang pasaporte

Sa ilalim ng bagong batas na Philippine Passport Act, ang mga kababaihan ay papapayagan na gamitin muli ang kanilang aiden ame sa kanilang pasaporte.

Ayon sa Department of Foreign Affairs-Office of the Consular Affairs (DFA-OCA), ang mga married women na magre-renew ng kanilang passport ay maaaring boluntaryong ipabalik ang paggamit ng kanilang maiden name.

Maaari itong gawin isang beses lamang ayon sa DFA at batay sa Section 5 ng Philippine Passport Act, pwede itong gawin kung ang existing ID cards ng passport holder ay maiden name din ang nakasulat.

Applicants who wish to avail of this mode of reversion to their maiden names in their passport may do so under the following conditions provided below:

Para sa mga kababaihan na nais muling gamitin ang kanilang maiden name sa pasaporte, kailangang isumite ang sumusunod:

– original and photocopy of a PSA-issued Certificate of Live Birth or PSA Report of Birth;
– original and photocopy of a PSA-issued Certificate of Marriage or PSA Report of Marriage;
– Notarized Affidavit of Explanation that includes a request for the reversion of maiden name in the Philippine passport or travel document and stating that the applicant has not hitherto availed of the reversion;
– latest-issued Philippine passport or travel document;
– any valid and existing Government-issued ID accepted for passport application, reflecting the maiden name of the passport applicant

Kung ang isang married woman ay annulled na, legally separated, divorce, o pumanaw ang asawa, may mga karagdagang dokumento pa na kailangang isumite a DFA. (DDC)
